Strategic area

Health

Care that can be completed, afforded and sustained

THESO’s proposed health approach would finance complete, clinically justified care pathways through qualified partners. Its starting point is the care a person needs through completion: primary, maternal and child health; indicated tests; essential medicines; and functioning referral. Clinical authorities would retain responsibility for individual treatment decisions.

Proposed approach

Define the whole care package

Country appraisal would compare local need with existing provision, then specify benefits, eligibility, lawful charges, exemptions and referral duties before enrolment. Affordability would include medicines, transport and the full course of care. Assistance for households unable to pay would be explicit; access would not depend on first accumulating personal savings.

Connect assessment to treatment

Prevention and assessment would proceed only where confirmatory diagnosis and appropriate care are available. Each stage would have a named owner and funded communication, transport and follow-up. Invitations would reflect staff, waiting-time, diagnostic and medicine capacity. Informed participation would remain voluntary, and evaluation would examine false results and potential harm.

Finance functioning services

Facility approval would require competent staff, safe premises, utilities, supplies and referrals supported by a complete operating budget. Credential checks, supervision, authorised procurement, stock control and referral agreements would sustain the service. Payments would distinguish readiness, routine operation and limited verified performance.

Protect information and verify completed pathways

Lawful custodians would control patient records, role-based access, corrections and disclosures. Offline procedures would protect care when connectivity fails; emergency care would not depend on digital registration. Independent reviewers would examine complete pathways, patient expenditure, exclusions and adverse events using necessary, lawfully accessed information.

Intended outcomes

The intended outcomes are more eligible care episodes completed within the authorised window and more people with a positive assessment reaching diagnosis and appropriate follow-through. Completion would be assessed alongside safety, affordability and inclusion; a narrower eligible population or rejection of complex cases would not count as improvement.

Protection and access

The proposed conditions include consent, confidentiality, respectful care, accessible registration alternatives and confidential complaints without retaliation. Serious incidents would trigger protection and independent clinical review. Any restriction of payment would preserve evidence and include a funded continuity or referral plan within authorised resources.

Long term direction

Public counterparts would build purchasing competence through completed operating tasks. External support would decline only after verified domestic payments and continued quality and affordable access. The long-term objective is sustained national capability; a timetable or training attendance alone would not establish readiness for transfer.

Who would do what

Proposed partner roles for Health
PartyRole
Public and clinical authoritiesDefine lawful benefits, eligibility and professional standards; retain clinical decisions and authorised purchasing duties.
Qualified delivery partnersOperate facilities, provide assigned stages of care, maintain supplies and protect patients and records.
THESOAppraise and administer proposed grants within its legal powers; finance agreed readiness, care and capability; obtain only necessary verification information.
Independent reviewers and communitiesReview clinical and spending evidence independently; patient accounts and complaints test access and remedy while protecting confidentiality.
